PLEASE NOTE:

1. A $35.00 transfer fee must accompany this form.

2. There is a $10.00 refundable deposit on each Key Fob issued.

3. Oaks North is a "Senior Community" therefore, an Owner/Tenant Age Verification Form must be completed including proof of age for each resident before Key Fobs will be issued.
